COTS ~ Client Outcome Tracking System was developed by a community collaborative of social service partners. The partners wanted to develop a case management tool that tracked more than demographics and service data. They wanted a system that allowed them to monitor and track the effectiveness of service outcomes. At the same time, they wanted a software that was flexible and customizable to their individual requirements. A software that worked for both the front line user and system manager.

Founding Partners

The Hamilton Housing Help Centre is a multi-service housing support agency providing leadership on housing related issues through its community partnerships and participation on various planning bodies in the municipality. The Hamilton Housing Help Centre is a client-centered organization committed to accountability and best practices in the delivery of services. In 2009 the Agency spearheaded the development of the COTS database – an integrated and accessible case management software. Hamilton Housing Help Centre envisioned the collection and analysis of standardized data in a single system. The system would allow users to get a true picture of the outcomes of service across the broader sector and serve as an effective planning tool to identify gaps and priorities. To bring this to reality Hamilton Housing Help Centre collaborated with DLD Business Support Services to design and develop an innovative case management software.

Expanding Partnerships

Partners wanted the ability to develop and share new modules with other providers. For research and evaluation purposed, they wanted a robust information sharing capabilities.

As COTS software developed, other partners joined and today it is used by a wide variety of social service providers in housing, children’s mental health and addictions services.

COTS was developed to help partners share information. In our communities, social support comes from a variety of both formal and informal support. To more effectively help clients, partners need to share information through integrated and responsive data management systems.

COTS continues to use a partner advisory committee to enhance its case management and respond to emerging trends and challenges in the social service sector.
